Terms and Conditions
Applicable for OCBC Cycle 2023 participants between the age of two (2) to seventy-five (75) years old.

Subject to the Great Eastern standard policy terms, conditions and exceptions.

All payouts will be based on assessments and approvals of duly completed claim forms together with supporting
documents within fourteen (14) days of the accident occurrence.

Claimants need to email the following supporting documents to Nonmotorclaims-sg@greateasterngeneral.com:
1. Completed Personal Accident Claim Form
2. Medical Tax Invoice/ Hospital Tax Invoice
3. Inpatient discharge summary report from the hospital (if there is hospitalisation)
4. Copy of NRIC

This Insurance plan is underwritten by Great Eastern, a wholly-owned subsidiary of Great Eastern Holdings Ltd
and a member of the OCBC Group.

This plan is protected under the Policy Owner’s Protection Scheme which is administered by the Singapore
Deposit Insurance Corporation (SDIC). Coverage for your policy is automatic and no further action is required
